Health Homes & Integrated Care
When we say we care for children holistically, we mean it. Our Health Homes and Integrated Care program staff collaborate with everyone in a child’s world—for example, her parents, teachers, and healthcare providers—to ensure they are working together for the child’s overall well-being. The Health Homes Program is for youth who have been diagnosed with two or more chronic conditions—such as depression and asthma—or with SED (serious emotional disturbance).
Our home visiting programs, including Youth Assertive Community Treatment (ACT), also work with young people who are at risk for psychiatric hospitalization or other high-intensity services, giving them a real chance to get the help they need while remaining with their families. Our extensive research and firsthand experience have shown us that with the right support, these children can—and do—go on to lead happy, healthy, and fulfilling lives.
Health Homes Program
The Health Homes Program is an innovative, optional plan for Medicaid-eligible individuals in New York State. A Health Home is not a physical location or home, but rather an innovative model for coordinating critical health care and other services to support an individual’s physical, mental, and emotional well-being.
Many children, adults, and families who require multiple services are doing well, but some struggle to find the services they need. The Health Homes model allows all of a person’s providers to communicate and share records so that services are not duplicated or neglected. Each client is assigned a Resource Coordinator who manages all of their services, as well as coordination and communication among care providers. The RC also makes certain the client’s needs are met in the first place—whether that means connecting them to therapy or ensuring their home has heat in the winter. Through the Health Homes Program, children, adults, and families receive regular care and services, which means fewer trips to the emergency room, less time spent in the hospital, and a healthier, happier individual leading a normal life.
Our program works with the Collaborative for Children and Families (CCF), Coordinated Behavioral Care (CBC), and Northwell Health, but we will work with healthcare providers of any affiliation to ensure the best care.
Home Visiting
At The Child Center of NY, we believe that all children — even those experiencing the most serious problems — deserve the chance to get the help they need while remaining with their families. For children at risk of psychiatric hospitalization, we offer 24/7 home-based crisis intervention, case management, and coping skills training through programs of varying intensity.

Youth ACT is an intensive, at-home intervention that brings treatment and services to youth with significant mental health challenges—and their families—so they can remain at home and together.
The Child Center of NY’s Youth Assertive Community Treatment (ACT) serves young people ages 10-21 who are at risk of entering or are returning home from, high-intensity services, such as a residential treatment facility. We welcome children with significant psychiatric needs who are at risk of institutional-level care, and we also support young people who have been in an out-of-home environment with their transition back to their family and community.
The Youth ACT program provides families with a comprehensive team of professionals—from psychiatrists to peer advocates—to deliver intensive, coordinated, and individualized services and therapeutic interventions to maintain the child in the home, school, and community. We use a strengths-based model that instills our clients with hope and goals for the future and a roadmap for getting there.
